The West Kent Dementia Roadmap provides high quality information about the dementia journey alongside local information about services, support groups and care pathways to assist primary care to support people with dementia and cognitive impairment, their families and carers.
The main audience is staff working in primary care, including GPs, nurses, dementia navigators and practice managers. The roadmap will also be of benefit to other professionals, people worried about memory problems, people living with dementia and their carers and families
People with dementia can also experience delirium
"Symptoms of Delirium (acute confusion) can easily be mistaken for a chronic memory problem, Dementia or Depression: In reality delirium may be the only sign of a significant medical illness and early identification is so important to ensure prompt treatment and return to wellness."
Dementia is an all-encompassing term for categorising a group of symptoms noted for a decline in a person’s mental ability to operate in an everyday environment. People diagnosed with dementia can experience extremely high levels of stress and a high sensitivity to the world around them. Examples of dementia include Alzheimer’s disease and memory loss.
